Cleveland Clinic is currently seeking a qualified Nurse Practitioner for a position in Internal Medicine at our Family Practice Office in Madison, OH. This position involves a mix of pediatric and adult patients, the majority of which are adults. Family Nurse Practitioners are preferred; however we will consider Adult Nurse Practitioners to see only the adult patients. Responsibilities include seeing new and established patients for both well/preventative care as well as sick/acute needs. The successful applicant would have their own schedule but would also assist the physician in addressing his patient’s lab results and refill requests. We would ask that the candidate commit to working at least one weekend shift per month (the office is open both Saturdays from 9am to 4pm and Sundays from 11am to 4pm). In addition, the office may have hours on holidays so the candidate would need to be open to working up to one holiday per year if need be.
Summary:
Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Provides advanced nursing care to patients with complex acute, critical, and chronic health conditions, including the delivery of acute care services. Practice with a variety of specialty-based populations including neonatal, pediatric, and adult. Practices in any setting in which patients require complex monitoring and therapies, high intensity nursing intervention, or continuous nursing vigilance within the range of high-acuity care.
Job Responsibilities:
* Assess health status by obtaining relevant health and medical history.
* Performs a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician.
* Identifies significant health and medical risk factors.
* Documents findings in medical record.
* Orders, interprets and evaluates laboratory test and other diagnostic studies to assess patient's clinical problems and health care needs.
* Develops a diagnosis by analyzing and synthesizing collected data.
* In collaboration with the physician, develops a treatment plan along with the patient and significant others.
* Performs diagnostic or therapeutic interventions consistent with certification, law and Cleveland Clinic privileges.
* May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing and CCF policy.
* Develops patient education plan.
* Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality, safe patient care.
* May perform admission history & physical, document progress notes, prepare discharge summaries as required.
* Assists with and facilitates discharge planning.
* Other duties as assigned.
Education:
Master's or Doctoral degree in Nursing preferred.
Experience:
Two years previous clinical nursing experience preferred. Licensure/Certification/Registration: Licensed as RN in applicable state, national certification as nurse practitioner by nationally accredited organization, current certificate of authority by applicable state Board of Nursing, certificate to prescribe preferred.
